Some of the key factors that influence your change initiative are:

Characteristics

Describe your organization/unit/program. How is it connected to other areas of the organization or network etc. Where is it situated (geographically, physical location, and even in relation to other related entities etc).

Networks and communication

Explain the organisation's communication and social networks, including formal and informal relationships among staff members. Culture: What are the organisation's shared values, beliefs, and norms, as well as the overall climate and receptiveness to change?

How ready is your organisation or work area for change. Is there any tension for change (groups within the system wanting changes includes patients, consumers, staff etc.). If there is no tension this is important information to have.

What resources or capacity for change are there, including the availability of staff, technology, and other necessary resources?
